Indiana Cohabitation Litigation Questionnaire

Consider this as very much of a work progress and I could apply this to more than litigation. It does cover the points for creating a cohabitation agreement without the detail. I think it covers the issues for which facts are needed if the couple go from living together to litigating against one another.

1. What is your income?

2. What is the other party's income?

3. Date you and other party started living together?

4. Date stopped living together?

5. Any joint bank accounts?

6. What joint debts?

7 Amounts of the joint debts.

8. Personal property acquired while living together

9. Real property acquired while living together

10 Intangible personal property acquired while living together

11. Debts of the other party paid for by you:

12. Debts of yours paid by the other party:

13. Your average yearly income while living with the other party:

14. The other party's average yearly income while living with you:

15. Where are you employed now?

16. If not employed, source of income:

17. List all places you were employed where you were employed while living with the other party?

18. Where it the other party employed?

19. If the other party is not employed, what is the other party's source of income?

20. List all the place where the other party was employed while living with you?

21. If you have a business, did you have that business while living with the other party?

22. Describe the business

23. If "yes", describe the other party's contributions and/or involvement with the business.

24. If the other party had a business while living with you, please give the following information:

A. Describe the type pf business.
B. Describe your contributions and/or involvement with the business.

25. Did you take out any loans while living with the opposing party?

26. Did the other party take out any loans in their name only?

27. How did you and the other party split your monthly bills? Especially who paid what monthly bills. If possible, the monthly amount of those bills.
